Enter a request for patient exchange once the patient has consented. We only need their postal code. Individual doctors simply use our free form, with bulk uploads offered on paid plans.

We work to find direct matches and three way trades. This way your distant patient can establish local care, and you take on a local patient in exchange. We’ll send you an email to notify you of an exchange opportunity.

Make the Connection
Now you can reach out to your colleague to confirm the exchange, inform your patients, and make arrangements to transfer medical records directly.
